These prices are effective as of November 1, 2011 and are subject to change without notice. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the goods and services you selected.
Basic Services of the Funeral Director and Staff	$1,850.00
Our services include: conducting the arrangements conference; planning the funeral; consulting with family and clergy; shelter of remains; preparing and filing of necessary notices; obtaining the necessary authorizations and permits; coordinating with the cemetery, crematory, or other third parties. In addition, this fee includes a proportionate share of our basic overhead costs. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)
Embalming	$745.00
Utah law requires embalming or refrigeration if the body is not buried or cremated within 24 hours from time of death. If a body is being taken out of state, being transported by common carrier, or died from a communicable disease, it must be embalmed by a licensed embalmer.
Other Preparations

Direct Cremation

Our charge for a direct cremation (without ceremony) includes: basic services of funeral director and staff; a proportionate share of overhead costs; removal of remains; transportation to crematory; necessary authorizations; and cremation if relevant. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). Any handling of the body other than removal (such as washing, disinfecting, and/or preparing of remains for identificaiton, dressing, or other use of staff and/or facilities) . . . . . $ 250.00
Immediate Burial

This discounted fee is available when a casket and vault are purchased from our funeral home. Forwarding of Remains to another Funeral Home	$1,500.00
Our charge includes: basic services of funeral director and staff; a proportionate share of overhead costs; removal of remains; embalming; other preparation of remains relevant to shipping; and local transportation to airport. Shipping will be in a minimum freight type container. This fee does not include services of staff or facilities for visitation, rites, or ceremonies prior to forwarding of the body.
Receiving of Remains from another Funeral Home	$1,100.00
Our charge includes: basic services of funeral director and staff; a proportionate share of overhead costs, care of remains, transportation of remains to funeral home and to cemetery or crematory. It does not include any visitation, rites, or ceremonies prior to the graveside. Additional services or use of facilities will be charged according to the general price list itemized fees.
